Los paraganglioma, son tumores de crecimiento lento altamente vascularizados, presentación entre tercera y quinta décadas de la vida. Son funcionantes en el 25 % -60 % de los pacientes. Se presenta caso de paciente femenino de 39 años de edad, con dolor lumbar punzante derecho irradiado a miembro inferior ipsilateral se solicita estudios de imagen TAC de tórax, abdomen y pelvis, evidenciando lesión de ocupación de espacio retroperitoneal, realizando múltiples biopsias las cuales inicialmente reportan negativas para malignidad. Acude a consulta 4 años después con TAC de abdomen y tórax, con evidencia de tu retroperitoneo y múltiples lesiones de ocupación de espacio hepáticos y pulmonares, se hacen revisión de biopsia e inmunohistoquímica compatibles con paraganglioma, concluyéndose como paraganglioma extra adrenal retroperitoneal no funcionante no resecable con metástasis hepática y pulmonar, recibiendo esquema de quimioterapia con respuesta clínica completa de lesiones hepática y pulmonar y parcial de lesión retroperitoneal haciéndose resecable.
The paraganglioma are tumors of slow growing and highly vascularized, with presentation between the third and fifth decades of life without a clear predisposition for either sex. They are functioning in 25 % – 60 % of patients. Case of female patient 39 years old, presents with low back pain radiating to the ipsilateral sharp right lower extremity CT imaging of the chest, abdomen and pelvis is requested, showing occupied space lesion retroperitoneal, performing multiple biopsies which initially reported negative for malignancy. Turn to 4 years after consultation with CT abdomen and thorax, with evidence of your retro peritoneum and multiple liver and lung occupied space lesions, review of biopsy and immunohistochemistry are compatible with paraganglioma, concluding as extra adrenal not funcionante retroperitoneal paraganglioma with un resecable liver metastases and lung, receiving chemotherapy regimen with complete clinical response of lung and liver injury and a partial becoming resecable retroperitoneal injury.
