OBJETIVO: El tuberculoma es un granuloma formado por la respuesta inflamatoria a la infección de tuberculosis, constituye una forma rara de manifestación, siendo su compromiso aislado extremadamente inusual. La presunción diagnóstica en casos sin antecedentes y compromiso aislado del SNC, en forma de tuberculoma, es extremadamente rara. Los tuberculomas intracraneales son la presentación menos común de la tuberculosis del SNC, representando el 1 %. Se diagnostican principalmente en función de las características clínicas, los cambios del líquido cefalorraquídeo y las características de imagen. En la espectroscopia en los tuberculomas se puede encontrar un importante pico de lípidos. CASO CLÍNICO: Se describe el caso de una escolar de 9 años quien cursó con clínica neurológica dada por cefalea holocraneana, vómitos y diplopía, con RMN cerebral que reporta LOE infra-tentorial planteándose diagnóstico de posible neoplasia del SNC por lo que realizan exéresis y biopsia de la lesión la cual reporta hallazgos compatibles con tuberculoma, los cuales fueron corroborados mediante características metabólicas en espectroscopia que sugieren dicha lesión, manteniendo tratamiento y control mensual con infectología y oncología. DISCUSIÓN: El tuberculoma es de diagnóstico clínico, serológico e imaginológico, representando una enfermedad extremadamente rara a cualquier edad y particularmente en pacientes pediátricos, la cual al realizarse un adecuado diagnóstico e iniciarse tempranamente el manejo y tratamiento correspondiente, mejora la oportuna determinación de diagnóstico diferencial con neoplasias incrementando la sobrevida de estos pacientes.
OBJECTIVE: The tuberculoma is a granuloma formed by the inflammatory response to tuberculosis infection, is a rare form of manifestation, being its isolated commitment extremely unusual. The diagnostic presumption in cases with no history and isolated involvement of the CNS, in the form of tuberculoma, is extremely rare. The intracranial tuberculomas are the least common presentation of CNS tuberculosis, representing the 1 %. They are diagnosed mainly according to the clinical characteristics, the cerebrospinal fluid changes and the imaging characteristics. In the spectroscopy in the tuberculomas you can find an important peak of lipids. CLINICAL CASE: The case of a 9 year old school girl who presented with neurological symptoms due to hole-cranial headache, vomiting and diplopia, with brain MRI that reports infra-tentorial LOE, being diagnosed of possible CNS neoplasm, is described, so they perform resection and biopsy of the lesion which reports findings compatible with the tuberculoma, which were corroborated by the metabolic characteristics in the spectroscopy suggesting said lesion, the patient maintaining monthly treatment and control with the infectology and oncology. DISCUSION: The tuberculoma is clinically, serologically and imaging diagnosed, representing an extremely rare disease at any age and particularly in pediatric patients, which when an adequate diagnosis is made and the corresponding treatment and treatment are started early, improves the timely determination of the differential diagnosis with neoplasia, increasing the survival of these patients.
